Italy's Nicolò Martinenghi won the silver medal in the men's 100 breaststroke at the World Swimming Championships in Singapore on Monday.
Qin Haiyang's took the gold medal with Kyrgyzstan's Denis Petrashov completing the podium.
Martinenghi's achievement was all the more remarkable as he was ill before the race.
"I don't know if I got food poisoning, but I spent the whole night in the bathroom," the Olympic champion in the event told Rai Sport.
"I thought about pulling out. But my heart was proud to represent Italy with the work I'd done.
"I could have done even better, but I'm still very happy.
"I knew my worth and I didn't want to waste this opportunity.
"I'm still over the moon. But now I can't stay on my feet." Another Italian, Ludovico Viberti, qualified for the final and came sixth.
On Sunday Carlos D'Ambrosio, Thomas Ceccon, Lorenzo Zazzeri and Manuel Frigo won the silver medal in the men's 4x100 freestyle relay at the World Swimming Championships in Singapore.
